6/20/2023 EnchantaVaughn Approved QMHP-A greeted Kaila upon arrival to the home setting. Upon arrival to the home setting, Kaila appeared to be in a content mood evident by her facial expressions such as a smile on the face and engaging in general conversation with QMHP-A. Kaila did not present any symptoms of S/I or H/I at the time of the session. QMHP-A inquired about Kaila’s current thoughts, feelings, and emotions. QMHP-A encouraged the use of the deep breathing exercises. QMHP-A used modeling to teach Kaila how to take three deep breaths, relax and in-hale. QMHP-A encouraged Kaila to concentrate on peaceful thoughts during the exercises. QMHP-A also educated Kaila on the symptoms of Bi-polar disorder/ Depressive Disorder. QMHP-A reinforced the importance of gaining knowledge through connecting with a therapist to help in reducing stress and stressful situations. QMHP-A probed Kaila regarding the impact of her inability to sleep has played on her start of the day. QMHP-A suggests identifying triggers that has caused her to be “stressed”. QMHP-A emphasized with Kaila the importance of managing medications based upon past and present experiences as well as directive from medication use given by her psychiatrist. Kaila appeared to be receptive to QMHP-A’s services and interventions that were provided to her during the session. Kaila shared that she did not sleep good last night because her neighbors were having a party and they were up until 5 this morning. Kaila shared she had a bad headache and needed to get out and get some fresh air. Kaila was able to process through weighing the pros and cons that she must be conscious of her actions because she does work, and she does need her job even though she does not like the new management and especially after she has learned that her daddy is working there. Kaila that she must set limits for herself as well as set boundaries and be more assertive in her decision making. Kaila also acknowledged that when communicating her needs she must learn to avoid stressful situations that may trigger her negative behaviors. Kaila admits that when she does not have her proper rest she will become easily annoyed by certain situations. Kaila indicated to QMHP-A she has been adhering to her medication management on a consistent basis and now she is just worried about the other stuff she has to deal with for a lifetime.
